To apply for affiliate status, existing UM faculty must send in:
1. A current comprehensive CV
2. A statement describing expected involvement with CCMB, and
3. Supporting letters from two current affiliates.

Normally, approval of affiliate status can be obtained in approximately two weeks after all required materials have been submitted.

To maintain affiliate status, faculty must annually submit an activities form.  This form can also serve as a guide to the type of activities to be included in the statement required in item 2 above.
